Perineal groove is a congenital malformation of the perineum characterized by a wet (nonkeratinized epithelium) sulcus spanning the region between the posterior vaginal fourchette and the anterior edge of the anus with a normally formed vestibule, urethra, and vagina. This case report describes a 1 year-old female infant with complains of the perineal wet sulcus (like fresh wound), the perineal sulcus was found to be extended from valvular genitalia to the anal opening.

It is unknown to many clinicians, which usually leads to misdiagnosis. The pathogenesis of perineal groove is not clear, and there are few cases reported in the current medical literature.

Although it may be self‐resolving during childhood, this nonepithelized mucous membrane can pose the risk of local irritation and infection, particularly urinary tract infection. Grooves in the leg and ankle bones for the tendons of the peroneus longus and peroneus brevis muscles. These grooves are found on the posterior surface of the lateral malleolus (the distal end of the fibula), on the lateral surface of the calcaneus bone, and on the plantar (inferior) surface of the cuboid bone. Perineal groove is a congenital anomaly of the lower perineum, whose incidence is unclear, although is believed to be higher since the condition might be underdiagnosed [1]. 2. Study design: Data on patients with PG managed at 10 participating Anorectal Malformation Network centers in 1999-2019 were collected retrospectively by questionnaire. The perineum in humans is the space between the anus and scrotum in the male and between the anus and the vulva in the female. The perineum is the region of the body between the pubic symphysis (pubic arch) and the coccyx (tail bone), including the perineal body and surrounding structures.
